Dr. Orlando V. Gonzalez is a designated Civil Surgeon certified by the USCIS thus approved by the United States Department of Homeland Security.

REQUIRED DOCUMENTATION
Please bring the following documents on the day of appointment:

A valid form of identification (such as a Driver's License or Passport)

Insurance card, if available
-
Vaccine Records (USCIS currently requires proof of COVID-19 vaccination)

The first two pages of the I-693 form printed out and filled out
HOW IT WORKS
Book an appointment by clicking on the "Email Request Appointment" or by emailing us at info@lifemod.com

Vaccination records are reviewed day of appointment.

Testing takes place on-site, the day of the patient's appointment.

The appointment will last approximately one (1) hour.

The patient will meet with the civil surgeon on the day of appointment.

The official I-693 form will be completed by the civil surgeon and provided in a sealed envelope for the patient to submit to the USCIS within 2 weeks of initial appointment date.
